Abstract

To improve clinical outcomes and extend life expectancy for Hispanic patients 65 years and older on warfarin therapy, involving adherence to medication, lifestyle, dietary modification, and reinforcement of the patient's knowledge regarding therapy. This is an improvement project on warfarin medication commonly used to reduce the risk of thromboembolic complications, and its efficacy is largely dependent on maintaining a therapeutic window. Because warfarin is affected by many other factors, patients need to be consistent with their medication compliance to minimize side effects. The educational intervention protocol will take place is an internal medicine ambulatory clinic. The inclusion criteria will include Hispanic patients who are on warfarin therapy and within the age group of 65 years old or older. 10 patients will receive the transcultural educational program and the other 10 patients will receive the standard educational material. Pre-questionnaires were given to the participants, the standard or the transcultural educational program was introduced by the DNP student, and post questionnaires were collected three weeks in a follow up appointment. The data collected was analyzed using a mixed ANOVA, analysis of variance. The analysis tested whether the change from the pre-test questionnaire to post-questionnaire was significant. Furthermore, it tested if the change differed by condition, the transcultural educational intervention versus the standard education material. The data showed that the educational intervention in the elderly Hispanic population proved effective. Proper education of warfarin therapy is an essential requirement for upholding a safe and efficient treatment to reduce side effects.
