Abstract
Healthcare transition (HCT) includes the preparation, transfer, and integration into adult healthcare. HCT is particularly important in adolescents with type 1 diabetes mellitus (T1DM) as they transition into adult-centered healthcare services. Currently there is no consensus on a universal approach to HCTs in this population; however, health organizations that implemented a HCT program using the Six Core Elements of HCT (6CEoHCT) as a framework have been favorable. In combination with the teach-back method of communication, this approach to a HCT could lead to favorable outcomes in the self-management of chronic conditions. This project aimed to provide clinicians with the education and training necessary to improve their knowledge, attitudes and behaviors when intervening and identifying adolescents with T1DM ready to begin the transition process. A total of 21 clinicians participated in the pre/post-intervention survey, attended an educational intervention, and implemented the teach-back method of communication and transition roadmap to facilitate the transition process. Results showed that there was a 69.9% positive change in the mean knowledge scores of participants, a 15.6% positive change in the mean attitude scores of participants, and a 34.9% positive change in the mean behavior scores of participants. Before the intervention, only 52% were aware of available tools and information to facilitate the transition process while another 43% were unsure. After the intervention, all participants (100%) indicated that tools to assess transition readiness were available to them. The findings illustrate that with proper education and training of clinical support tools and the 6CEoHCT, clinicians are more open to transition planning.
